Located in the heart of the recovery community in St. Paul, Minnesota, Transitions offers a full range of recovery services for individuals who are drug and alcohol dependent.
With a decade of experiencein transitional living programs,our nine sober living houses and the more structured residential program Cornerstone provide three keys to successful recovery:
A low staff to resident ratio to build confidence, maintain accountability, and encourage fellowship.
A balanced program, tailoredto the individual, with enough structure to help residents develop new life skills and living habits while giving them time for reflection and self discovery.
A close-knit community, including not only the immediate residential household but the larger community of Transitions residential houses located within a half mile of each other in St. Paul. This provides our residents with more opportunities to form lasting relationships with others in transition who may have complementary interests.
We also provide a more specialized residential program for individuals with co-occurring mental illness and chemical dependency at our Foundation House, which qualifies for funding under the Rule 25 guidelines.
Our Transitions Outpatient Program provides treatment and continuing care services that include individualized treatment and aftercare planning.
Transitions also offers assessment, individual and group counseling as well as education for clients and their families.
